在美國藥局的英文

美國藥局不只是抓藥的地方。它同時是接種疫苗的小型診間、簡短醫療問題的諮詢櫃檯，以及一間擺滿 OTC（over-the-counter，無需處方）商品的店。對新移民與訪客來說，櫃檯的對話節奏可能很快，而且充斥不熟悉的詞彙：copay、prior authorization、generic、refill、controlled substance。櫃員開口的第一句話可能就是問你的 date of birth（出生日期）。藥師可能想了解你還在吃哪些藥。能用英文聽懂這套例行流程，整趟就會順很多。

你會看到什麼

典型的美國藥局有兩個窗口：drop-off window（送單窗口）讓你交處方，pick-up window（取藥窗口）讓你領備好的藥。許多藥局把兩個窗口合併成同一個櫃檯，再問你要送單還是取藥。

到櫃檯時，店員通常會先問你的全名與出生日期以調出檔案。第一次來的病人可能會被問到地址、電話、insurance card（保險卡），以及過敏史。新處方從備藥到完成，可能要 15 分鐘到幾小時不等，視藥局忙碌程度與藥品是否現貨而定。例行的 refill 可以透過藥局 App 或網站送出，再去櫃檯領袋子。

藥師本身是受過訓練的醫療專業人員。你不用預約就可以向他們詢問關於某個藥的簡短問題。許多藥局還有獨立的 immunization 櫃檯或椅子，可在那裡施打 flu shot（流感疫苗）或其他例行疫苗，有時不需預約，但政策因地而異。

常見費用、政策與文件

價格與規則差異很大。同一種藥在不同藥局售價可能差很多，加上保險計畫又改變整體情況。永遠不要憑直覺猜，先問清楚再說。

Copays。 多數保險計畫下，你會在櫃檯付 copay。金額視保險計畫與藥品而定。學名藥版本通常比品牌藥便宜。如果金額看起來奇怪，可請藥師重新跑一次保險請款，或查 cash price。
Cash price 與 insurance price 比較。 有時 cash price（不走保險）反而比保險的 copay 還低，尤其是便宜的學名藥。可以問：「Is the cash price lower than my copay today?」
Prior authorization。 某些藥品需要保險公司事前核准。藥局通常會說「the doctor's office needs to call in a prior authorization」。這個步驟可能要好幾天，務必跟診所確認他們已收到請求。
Controlled substances。 部分藥品（例如某些止痛藥、安眠藥、特定 ADHD 用藥）有額外規定。你可能需要出示政府核發的照片證件。續領次數可能有限，提前續領常被拒絕。實際規則請跟藥師確認。
30 天藥量與 90 天藥量。 許多保險計畫允許例行用藥開 90 天份，每顆單價可能更便宜。可詢問藥師能否改成 90 天份。
取藥保留期限。 已備好的藥通常會保留一段時間，超過就會退回庫存。如果你無法準時來領，先打電話請藥局多保留幾天。
身分證件規定。 某些處方藥，尤其是受管制藥品，藥局會要求看照片證件。能帶就帶。

You: Yes, I've taken it for about six months.

Tech: Great. Any questions for the pharmacist today?

You: Actually, yes — could I ask about taking it with my new vitamin?

Tech: Of course, I'll let her know.

保險在櫃檯被拒：

Tech: Your insurance came back saying it needs prior authorization.

You: I'm sorry, what does that mean exactly?

Tech: Your insurer wants your doctor's office to send in a form explaining why you need this specific medicine. Until they do, the insurer won't cover it.

You: How long does that usually take?

Tech: It can be one to three business days, sometimes longer.

You: Could you let me know what the cash price would be in the meantime, in case I need to start it sooner?

Tech: Sure, give me a moment to look that up.

向藥師詢問新的藥：

You: Hi, I just picked this up. Could you tell me a little more about how to take it?

Pharmacist: Of course. This one is taken once a day with food. Take it at roughly the same time each day.

You: Any common side effects I should watch for?

Pharmacist: Some people feel mildly nauseous in the first week. If you have severe stomach pain, a rash, or trouble breathing, please call your doctor.

You: I also take a multivitamin and an antacid in the evening. Is that okay?

Pharmacist: Try to separate the antacid from this medicine by at least two hours. The vitamin is fine.

OTC 商品的諮詢：

You: Excuse me, my throat is sore and I have a dry cough. I'm not sure which one to choose.

Tech: Are you also stuffy, or just the throat and cough?

You: Just throat and cough.

Tech: Then you probably don't need the all-in-one cold medicine. A throat lozenge plus a simple cough suppressant might be enough. Any other medicines you take regularly?

You: One blood pressure medicine.

Tech: Let me grab the pharmacist quickly to make sure there's no interaction.

實用小提醒

第一次去某家藥局，記得帶照片證件與保險卡。一份目前在吃的藥、維他命與保健食品清單也很有用。
領取新藥時，當藥師主動提供諮詢，請接受。對話通常很短，但很值得。
同意品牌藥之前，先問是否有學名藥版本。
若 copay 高得意外，請藥師比較保險價與 cash price。
把藥局的電話存起來。你通常可以用電話請求續領、詢問藥是否備好、確認營業時間。
不要自己停藥或改劑量。覺得不對勁時，打電話給開藥的醫師。
把藥師當成資源，OTC 商品的小問題也可以問。不用預約。
