「Are you taking any medications?」というフレーズは何度も耳にしたことがあるはずです。けれど、「毎朝 5 mg の降圧剤を食後に 1 錠飲んでいます」と自然な英語にしようとすると、ふと言葉に詰まってしまいます。服薬歴の核心は実は 4 つだけ — 薬名、用量、頻度、服用タイミング。この 4 つの欄を埋められれば、ほとんどの医師・薬剤師の質問に答えられます。
