Medical History Questionnaire

Kuesioner biasanya berupa daftar panjang yes/no, contohnya:

Have you ever had: high blood pressure / diabetes / heart disease / asthma / cancer / stroke / depression …

Centang sesuai kondisi sebenarnya. Klinik bertanya untuk keamanan obat, bukan untuk melabel kamu.

Allergies

Biasanya dibagi tiga:

  • Drug allergies (alergi obat)
  • Food allergies (alergi makanan)
  • Environmental allergies (alergi lingkungan: serbuk sari, debu, hewan peliharaan)

Tiap alergi biasanya disertai kolom reaction (reaksi). Kamu bisa isi rash / hives / swelling / trouble breathing / nausea.
